CHW Application Process
How do I apply to become a Certified CHW?
To apply for CHW certification, follow the steps below:
- Create an account here.
- Choose the pathway that is right for you. See the CHW Pathways page for more information
- Complete the application
- Submit the application

- Find steps to create an account here.
- Applications do not need to be completed in one sitting. Applicants may save and exit an in-progress application and then return to it later to provide more information or upload required documents.
- You will need to be able to upload required documents to the application before it is submitted.
- Use your personal email address in your application. If you are not receiving emails from the IL CHW Certification Program, check your spam or junk mail folder.
- After you submit your application, IDPH will send you an email to confirm it was received. If more information is needed, IDPH will contact you by email. Once your application is complete, IDPH will review it and contact you by email within 60 calendar days to let you know if it is approved.
Background Check Requirements
Is a criminal background check required for CHW certification?
All CHW applications ask the applicant to disclose felony convictions. If an applicant discloses a felony conviction, they can provide information to support their application, such as a letter attesting to their character. Further information can be found on the FAQ page.
